Apis mellifica: keynotes and uses
Apis mellifica — the common honey bee, prepared from the whole insect — is one of the earliest remedies a student of homeopathy meets, because its keynote picture maps directly onto something immediately recognisable: the hot, puffy, stinging swelling of a bee sting. It is reached for when a complaint is marked by rosy oedema, burning and stinging pains, and a striking relief from cold applications.

Apis mellifica is prepared from the honey bee, Apis mellifera, an insect of the order Hymenoptera whose venom contains melittin, phospholipase, and histamine-releasing peptides responsible for the pain and swelling of a sting. The remedy is made from the whole live bee or from the venom, then potentised by serial dilution and succussion in the standard Hahnemannian or Korsakovian methods. The doctrine of similars is unusually legible here: the crude sting produces exactly the burning, stinging, oedematous local reaction that the remedy picture describes.
Apis entered the materia medica in the nineteenth century through Hering's early American provings, with Boericke and Kent later expanding the keynote picture from their casebooks. It holds a stable niche: states of rapid, rosy, watery swelling with burning and stinging pains, and a constitutional layer of restlessness and irritability.
Keynotes
Six features recur in Boericke, Hering, and Kent with enough regularity to serve as a working checklist.

Boericke captures it concisely: "burning, stinging pains," "oedema," and "thirstlessness" in a patient who is "fidgety" and worse from warmth. The deciding combination is the relief-from-cold modality paired with the burning–stinging–puffy triad: a swelling the patient wants cooled rather than warmed.
Indications
Apis is indicated for the local reaction to insect stings, puffy swelling of the eyelids and face, urticaria with burning and itching, and oedematous states with thirstlessness and relief from cold. Apis 30C or 200C is the usual range — give, observe, reassess rather than redose on a fixed schedule.
Further indications documented across the materia medica:
- Acute urticaria and hives — raised, rosy weals with intense burning and stinging, better from cold bathing.
- Conjunctival and facial oedema — puffy, watery swelling around the eyes, worse from heat.
- Febrile states with thirstlessness — fever with scanty urine and absence of thirst.
- Meningeal irritation — when the picture carries the oedematous, stinging character; Hering documents sudden shrieking or cri encéphalique in this context.
- Ovarian and pelvic inflammation — right-sided predominance noted by Kent, with bearing-down and stinging pains.
- Dropsical conditions — general oedematous states with scanty urine, worse from heat.
Right-sidedness of complaints, sudden onset of swellings, and aggravation from heat in any form are the consistent threads.
Modalities
Worse: heat in any form (warm room, warm applications, warm drinks), touch and pressure, late afternoon (around 3–5 pm), after sleep, in closed heated spaces.
Better: cold applications, cold bathing, open air, uncovering.
Kent treats the heat aggravation as one of the primary differentiating points of the remedy — a patient who cannot tolerate warmth and is systematically relieved by cold.
Mental and general features
The Apis patient is restless and fidgety, difficult to please, and may present with jealousy, weeping without apparent cause, or a busy, fretful state that runs alongside the physical complaint. Kent and Hering both treat this mental layer as an important confirmatory dimension rather than a peripheral note.
Thirstlessness is a general of the first rank. Its presence in a febrile patient with oedema strongly lifts Apis above its competitors.
Differential — Apis, Belladonna, Urtica urens
Apis sits closest to Belladonna and Urtica urens, because all three present in red, hot, or stinging pictures.

The relief-from-cold and the pitting oedema separate Apis from the dry heat of Belladonna; the deeper systemic involvement and thirstlessness separate it from Urtica urens, which usually stays confined to the skin.

Clinical awareness — when not to prescribe alone
Stings and allergic swellings can present as airway emergencies. Swelling of the lips, tongue, or throat, difficulty breathing, a spreading rash with faintness or collapse, or any reaction following a sting in a person with a known sting allergy are red-flag presentations needing prompt assessment and, where prescribed, adrenaline. This is true of any acute oedematous reaction, especially in children, older patients, and anyone with a known anaphylactic history — the remedy work runs alongside that recognition, not in place of it.
